A shemale is generally a transgender or transsexual person. That is someone who either undergoes hormone therapy or surgery to change their gender. The word "shemale" is a word generally used only in porn or in a sexual context. It can be considered derogatory. Usually shemales are pre-op. Meaning they haven't had surgery/ vaginoplasty and therefore still have a penis.
